陰唇反覆疼痛 誤認潰瘍 罹貝西氏症 及早治療以免惡化 【中央社】一名21歲的年輕少女幾個月來陰唇疼痛，誤以為潰瘍，診治發現罹患「貝西氏症」，經過三個月的治療，病情獲得改善，追蹤八個月無異狀，以後只要定期追蹤即可。 台灣童綜合醫院免疫風濕科主治醫師陳國書表示，貝西氏病是一種侵犯多重器官的炎症，會有口瘡、生殖器瘡、關節炎、皮膚病症、眼睛、腸胃道及神經多個系統的病症。 貝西氏症的主要特徵除反覆口腔潰瘍，很多患者會合併陰部潰瘍、虹彩眼、結節性紅斑、關節炎、假性挫瘡，有些甚至會有中樞神經或大小腸潰瘍，症狀輕重不一。 單純口腔潰瘍在一般人身上也常會發生，但如果合併上述症狀就可能是系統性的免疫失調症狀，若能早期診斷通常治療效果很好。 這名年輕少女的陰唇出現疼痛感已有一段時間，病況時好時壞，並不斷的反覆發作，於是到婦產科看診，一開始先排除癌病變。 由於患者陰唇疼痛仍存在，且潰瘍也反覆發作，於是轉診至免疫風濕科，發現她的小陰唇已蝕穿成一個洞，比女生穿耳洞還大約0.5公分，追問病史得知嘴巴常有潰瘍現象。 患者雖然無其他明顯特徵，醫師依臨床經驗判斷罹患「貝西氏症」，用藥治療後，病情逐漸獲得控制，並追蹤一段時間，無復發情形。貝西氏症男女皆可能發生，為何會產生目前病因不明，此病大多以臨床診斷，另外在病理上可看出嗜中性白血球過度活躍的現象，可幫助醫師診斷。 陳國書說，一般貝西氏症的症狀，一定要及早治療，以免惡化，造成嚴重的後遺症，若出現類似的症狀，一定要尋找專業醫師的協助。 2008-08-14
