台北醫學大學附設醫院婦產部內視鏡中心主任鄢源貴說明，問題就出在卵巢「偷懶」，卵子如果無法成熟排出，大小不等、至少十個以上的小囊泡累積在卵巢中，就形成「多囊性卵巢」。時間一長，這些小囊泡會分泌雄性荷爾蒙進入血液中，引起上述症狀，稱為「多囊性卵巢症候群」，視個人狀況，症狀可能輕重不一，也有人完全沒有症狀。
鄢源貴表示，過去研究認為，與性荷爾蒙有關的卵巢、腦下垂體、下視丘都可能是多囊性卵巢的影響因素。但1980年之後的研究顯示，「胰島素阻抗」是造成雄性荷爾蒙過高的原因，即患者的身體細胞對胰島素利用有缺陷，因此胰臟細胞代償性分泌更多胰島素，造成血中胰島素量比正常人高，使卵子周圍的濾泡細胞分泌更多雄性荷爾蒙，或是使肝臟分泌性荷爾蒙結合蛋白減少，相對使血液中活性的雄性荷爾蒙增加。
因此，近年醫師利用治療糖尿病的口服降血糖藥，來增加組織對胰島素的敏感性，幫助清除過多的血糖，鄢源貴表示，這是從改善體質的方式著手。
由於不排卵而無法生成黃體素，子宮內膜不會定期剝落，而持續的子宮內膜增生，容易造成子宮內膜癌。因此，不打算生育的婦女，可服用避孕藥降低雄性荷爾蒙，維持月經規律性，或是定期以黃體素催經調整。由於多囊性卵巢患者排卵不易，如果想要懷孕者，可以排卵藥促成懷孕。
鄢源貴建議兩者並用，一方面改善症狀，另外再以降血糖藥改善體質，既治本也治標；嚴重者，也可以腹腔鏡合併雷射或電燒手術，將多囊性卵巢的囊泡燒掉，但療效並不長；若為多毛、痘痘困擾，可考慮接受皮膚科美容治療。
而多囊性卵巢合併肥胖者，也容易罹患糖尿病、高血脂、高血壓或是心血管疾病。鄢源貴比喻，多囊性卵巢與肥胖，就像蛋與雞的因與果關係，胰島素增高會讓脂肪容易堆積，而細胞周邊脂肪也易轉化成雄性荷爾蒙，因此除了藥物治療，也必須控制體重、減少碳水化合物攝取，多吃含纖維食物。
